Underpainting

Before I go further with this underpainting I am hoping for a little help with the background. The reference photo was taken in the hospital and therefore there is nothing to draw on. Just a white wall. The white pillow that the infant is laying on seems like plenty of white already. Should I add a window? A drape? I would like the focal point to be the four hands. In the photo they are lighter than in my depiction here.
Any help is appreciated.

Edited to add; In resizing this file to fit on this site I chopped off half of my piture. It is 18 x 12 horizontal (not vertical)

Can you get Cynthia to help you post your whole painting? I think it is important to get an accurate picture.
